Тревога и депрессия у пациентов с туберкулезом в Кыргызстане: распространенность и взаимосвязь с возрастом и длительностью лечения

Аннотация

Введение. Туберкулез остается значимой проблемой здравоохранения в Кыргызстане, а тревога и депрессия являются распространенными коморбидными состояниями, влияющими на приверженность и исходы лечения. Однако данные о психоэмоциональном состоянии пациентов с туберкулезом в стране ограничены. Цель исследования. Оценить уровни тревоги и депрессии у амбулаторных пациентов с туберкулезом легочной локализации и определить их взаимосвязь с возрастом и длительностью терапии на основе шкалы HADS. Материалы и методы. Проведено поперечное аналитическое исследование 401 пациента, проходившего амбулаторное лечение в четырех областях Кыргызстана. Уровни тревоги (HADS-A) и депрессии (HADSD) оценивались с использованием шкалы HADS. Данные анализировались по возрастным группам (18–29, 30–49, ≥50 лет) и длительности терапии (<4, 4–12, >12 месяцев). Применялись непараметрические методы (Краскел–Уоллис, χ² Пирсона), клинически значимая симптоматика определялась при пороге ≥8 баллов. Сбор данных осуществлялся с использованием платформы KoBoToolbox, статистическая обработка и графическая визуализация — в программной среде R 4.4.3 (2025-0228, ucrt) с использованием интерфейса RStudio 2025.09.2 (Build 418) (Posit Software, США). Результаты и обсуждение. Повышенная тревога (≥8 баллов по HADSA) выявлена у 29,4 % пациентов, а повышенная депрессия (≥8 баллов по HADS-D) — у 39,4 %. Уровень тревоги был статистически выше в группе 30–49 лет (p=0,035), депрессия — у пациентов ≥50 лет (p=0,0098). Длительность лечения не влияла на уровень тревоги, однако для депрессии отмечена пограничная тенденция к росту при терапии более 12 месяцев. Заключение. Тревога и депрессия широко распространены среди пациентов с туберкулезом, причем депрессия чаще наблюдается у пациентов старше 50 лет, а тревога — у пациентов среднего возраста. Данные подчеркивают необходимость рутинного скрининга психического здоровья и усиления психосоциальной поддержки в программах амбулаторного ведения туберкулеза в Кыргызстане.
